To install this plugin.

1. Tools -> Options
2. Plug-ins -> Install
3. Locate and select the PerforationCreator.rhp
4. The command should be PerforationCreatorCommand, this can be verify by checking Plug-in Properties
5. Create a new button on the tool bar to run that command, can import the image of the icon.
